"Heart-Shaped Glasses (When the Heart Guides the Hand)" is the sixth track on the 2007 release Eat Me, Drink Me and the first single. Initially, "Putting Holes in Happiness" was set as the first single from the album, but this was changed, similar to the case of "Get Your Gunn".

The digital single was released on iTunes on April 24, 2007, and also added to the player on Manson's official MySpace page on the same date. On May 22, 2007, an exclusive single was released through Hot Topic, containing a different b-side than the other singles ("You and Me and the Devil Makes 3"). The CD single was released in Germany on May 25, 2007, and in the United Kingdom on May 28, 2007.

On April, 27, 2007 a 34 second clip and a 59 second clip of the video for "Heart-Shaped Glasses (When The Heart Guides The Hand)" were leaked via YouTube.

On May 23, 2007, Marilyn Manson and Tim Skold performed an acoustic version of the track on UK radio station Radio 1, along with the Justin Timberlake cover "What Goes Around.../...Comes Around".

The music video, directed by Marilyn Manson using James Cameron's 3D camera (wrongly believed by some to have been directed by Cameron himself) begins with clips of Manson and Evan Rachel Wood having sex (although the footage is not explicit in any way) with the song "Evidence" playing in the background. After the short sex scene is over, Manson and Evan are shown in a car driving while Manson is taking pictures of Evan posing with a butcher knife. After this, the song begins to play, and Manson is shown performing in front of a crowd in a dimly lit room; Evan stands in the midst of the dancing crowd, literally wearing heart-shaped glasses. The performance is interspliced with scenes of them driving and having sex while blood rains down upon them. At the end of the song, they are shown again driving the car as they utter the words "Together as one - against all others", right before driving off a cliff. The complete music video is over seven minutes long; however, the more common edited version of the video is barely over five minutes, and does not include the lengthy intro. Manson is quoted by the Insider regarding Evan's paycheck for the video appearance " I did insist that Evan be paid the most that any actress has ever been paid in music-video history to be in this, even though she wouldn’t have asked for it. There’s no one else that could’ve been in it, because it was inspired by her.”

  • According to Sean Beavan Manson initially wanted "Putting Holes in Happiness" to be the first single though the record company decided that "Heart-Shaped Glasses (When the Heart Guides the Hand)" was a more natural choice due to its radio friendly nature. But as speculated, the song was the second single for the album was indeed "Putting Holes in Happiness".
  • Manson wrote "Heart-Shaped Glasses (When the Heart Guides the Hand)" after 19-year old girlfriend Evan Rachel Wood came to his house wearing heart-shaped glasses as seen on the movie poster for Stanley Kubrick's 1962 film, Lolita.
  • The main guitar riff resembles that of the David Bowie song "Ashes to Ashes" from his 1980 album Scary Monsters. It is also noteworthy that Manson stated he had listened to Scary Monsters quite a few times while recording Eat Me, Drink Me.
  • The song's working title was simply "When the Heart Guides the Hand".
